(immediate corrections: removal of plastic bags). 0165(4)(f) Licensor observed there was no safety gate at the stairway to prevent children from having access to the unlicensed rooms upstairs. 0165(5)(d) Licensor observed the TV was plugged into a surge protector hanging from the wall that was accessible to children.
